COVID-19 Update

COVID-19 UPDATE       June 12, 2020

Hello!  Know that we are excited to be back to serve you.  It is of utmost importance for us to do so in a safe manner.  

There are many new precautions in place. We appreciate your cooperation ensuring that all of the necessary steps are carried out.

  • All visitors entering our clinic will be screened for COVID-19. If a patient of the clinic requires a companion, only one companion is permitted, they will be screened as well.
  • Hand sanitizer is available throughout the clinic.  We ask that you use it upon entering and as needed throughout your visit.
  • The Ministry of Health strongly urges you to bring and wear a mask.  For some appointments it is a requirement.  Please consult your practitioner for more information.

We ask that you do not enter the clinic if you are experiencing COVID symptoms. Kindly contact us by phone.

Please self screen in the following manner.  You will be required to complete the same screening in a documented manner when you arrive.

Please help us ensure your safety and the safety of the practitioners and public by answering the following questions:

1. Have you had close contact with anyone with acute respiratory illness or travelled out of Ontario in the last 14 days?

2 .Have you had a confirmed case of COVID-19 or had close contact with a confirmed case of COVID-19?

3. Are you experiencing any of the following symptoms:

-Fever
-New onset of cough
-Worsening chronic cough
-Shortness of breath
-Difficulty breathing
-Sore throat
-Difficulty swallowing     
-Decrease or loss of sense of taste or smell
-Chills
-Headaches
-Unexplained fatigue/malaise/muscle aches (myalgais)
-Nausea/vomiting, diarrhea, abdominal pain
-Pink eye (conjunctivitis)
-Runny nose/nasal congestion without other known cause

4. If you are over 70 years, have you experienced any of the following symptoms: delirium, unexplained or increased    number of falls, acute functional decline, or worsening of chronic conditions?

 At present, appointments can be made to see Dr. Zeinin Haji. DC and Dr. Amy Crinklaw DC.

All other practitioners are either open or working towards opening.  Virtual appointments are still available by those who offer them.  You are welcome to connect with them directly through the contact information provided on their bio page on our website or by contacting 519 652-5597.

Please be patient as there are a number of changes and that are required on our part and things are changing daily with directions from our governing bodies.  It’s a learning curve that all of us need some time to get used to!
